Northern Health Care Management

Northern Health Care Management provides comprehensive services for persons who are nursing home eligible and choose to receive care at home, or for those who are living in a nursing facility and wish to return to or remain in the community, in a setting of your choosing.

Through the assistance of our Supports Coordinators’ assessment of the individual’s needs, we tailor services and supports for your unique situation.
